I hereby declare in accordance with the provisions of section 96a of the Mining Act 1978 that the undermentioned mining tenement is forfeited for failure to comply with statutory requirements being failure to lodge the annual form 5 operations report. This notice comes into operation at 12:00 midday on the day of publication.
