(1) Can the Minister confirm that around 11.00am on February 4 2003, Senior Environmental Waste Water Officer, Ms Julie Phelps of the Health Department, visited a property located in Freeman Road, Forrestdale and interviewed the resident of that property?
(2) If yes, can the Minister advise the purpose of Ms Phelps’ visit?
(3) Can the Minister confirm that Ms Phelps made a number of pages of handwritten notes whilst on the property?
(4) Can the Minister confirm that the subject matters on which Ms Phelps made her notes largely related to the health of the resident’s family and conditions on the property?
(5) If yes, can the Minister table any reports, handwritten notes or documents arising from Ms Phelps visit to the property?
(6) If not, why not?
(7) Can the Minister advise whether any subsequent correspondence was entered into between either Ms Phelps and the resident, or between any other officer of the Health Department and the resident, on issues relating to either the resident’s health or the resident’s property?
(8) If yes, will the Minister table copies of all such correspondence?
(9) If not, why not?
