parliamentary secretary representing the Minister for Mines and Petroleum:
I ask this question on behalf of Hon Robin Chapple who is
away from Parliament on urgent parliamentary business, and which makes
reference to question without notice 198 regarding the Harlequin mine and the
new Sir Major decline.
(1) At what time
and date was the Department of Mines and Petroleum first advised of the fall of
ground or fatality in the 203 east drive at the Sir Major decline?
(2) How many
notifications were there of the fall of ground or fatality in the 203 east
drive at the Sir Major decline?
(3) With all
notifications, will the minister advise who made those notifications and in
what chronological order they were made with the associated dates and times?
(4) To which DMP offices were those
notifications made?
(5) If no to any of the above, why
not?

question.
(1) The first
notification of the event was made to the Department of Mines and Petroleum's
district inspector of mines at 1245 hours on 15 February 2014. This call was
not received at the time as the district inspector was travelling to Perth and
out of mobile phone range.
(2) There were
three notifications on 15 February 2014. The first notification was the same as
in the response to (1). The second notification of the event was received by
the Department of Mines and Petroleum on-call regional inspector of mines via
Worksafe and Police at 1625 hours on 15 February 2014. The third notification
was via another mobile phone message notification of the event to the district
inspector of mines at 1641 hours on 15 February 2014. It was not received as he
was out of mobile phone range.
(3) Notifications
to the district inspector of mines were from Nigel Driscoll, mine foreman with
Central Norseman Gold Corporation Ltd, and were made at 1245 hours and 1641
hours on 15 February 2014. Notification to the on-call regional inspector of
mines was from Worksafe at 1625 hours on 15 February 2014.
(4) Notification
was to the district inspector of mines for the Kalgoorlie office and to the
Perth-based on-call regional inspector of mines.
