The Driver was interviewed by his immediate Manager where it was ascertained that he had mistakenly thought he was running a different pattern service (ie. non stop). (d) An incident of this sort occurs very rarely. The Public Transport Authority estimates it would happen about once in every 50 000 services. At the commencement of each shift Train Drivers are informed of the stations they are required to stop at and that information is keyed into the on-board computer system which activates the audio message system. Train Drivers have been de-briefed to ensure that this type of incident does not occur again. Repetition of such an error by the driver would lead to reassessment, additional training and then the formal performance appraisal process, potentially leading to dismissal (e) Not applicable.
