(b) what was the total value of the mobile phones that were lost or stolen;
(c) was the loss or theft of any of these mobile phones reported to the police;
(d) if yes to (c), when were these reports made;
(e) of those reported, what has been the outcome;
(f) if any mobile phones that were lost or stolen were not reported to the police, why not;
(g) what steps, if any, have been put into place with a view to eliminating, or at least reducing, these losses; and
(h) when were these steps put into place?

DEPARTMENT OF THE ATTORNEY GENERAL
(a) Two mobile phones were stolen and no mobile phones were lost.
(b) $1,305 excluding GST.
(c) Yes.
(d) Police reports were made for the two stolen mobile phones on the following dates:
- 29 March 2007; and
13 April 2007.
(e) The items reported have not been recovered.
(f) Not applicable.
(g) The Department has a policy that requires staff to ensure the security of mobile phones and also requires that stolen and lost mobile phones be reported to the Police. The Department requires the person reporting the incident to sign a Statutory Declaration outlining the facts surrounding the loss or theft of the mobile phone and for Managers to consider the degree of staff negligence involved in the loss of the item. Where it is determined that an individual was negligent, disciplinary action may proceed.
(h) Reporting lost mobile phones to the Police was introduced in August 2006. Completion of Statutory Declarations and consideration of any negligence involved in the loss was introduced in December 2006.
DEPARTMENT OF HEALTH
(a) 44 mobile phones and one satellite phone.
(b) Combined total value of $8,351.00
(c) Yes. Eight stolen mobile phones and one stolen satellite phone were reported to the police. In addition, although it is not Department of Health policy to report lost mobile phones, one lost mobile phone was also reported to the police.
(d) The thefts were reported immediately after it was ascertained that the items were stolen, not lost. The reports were made on:
- 25 January 2007 (1 mobile phone;
- 29 January 2007 (1 mobile phone);
- 1 February 2007 (1 mobile phone);
- 29 April 2007 (3 mobiles and 1 satellite phone);
- 12 May 2007 (1 mobile phone);
- 14 May 2007 (2 mobile phones); and
- 27 May 2007 (1 mobile phone).
(e) One lost mobile phone was found. None of the mobile phones reported to the police have been recovered.
(f) Mobile phones known to be lost, not stolen, were not reported to the police. Some phones stolen were old and of a very low value and were not reported to police.
(g) The Department of Health's Information Technology Policy No. P11/0804, titled
Mobile Telephony Policy
, instructs all Western Australian Government Health Sector entities regarding the allocation, control, use and security of mobile phones. Individual hospitals have their own procedures based upon this policy.
