(1) There is no current document in WA that is labelled as an action plan
for dementia. WA Health has developed a WA Dementia Model of Care 2011
which is a guide for health and related services for best practice in dementia
care. In addition, WA is a participant in the National Framework for
Action on Dementia (NFAD) 2006 - 2010 (extended to 2013).
A new NFAD is currently being developed by the national Community Care
Population Health Principal Committee (CCPHPC) Dementia Working Group. WA
Heath is actively represented on this national working group.
(2) Key elements of the WA dementia work include:
- Consultation and Collaboration - a WA Dementia Working Group with
representation from the Carers Association WA, Health and Consumers Council,
Alzheimer's Association WA, specialist medical and hospital professionals,
community support services, and areas of education, training and research.
- Mapping of existing service provision to gain a better understanding of
current services, both specialist and generic, that deliver service to people
affected by dementia.
- Identifying good practice models in Western Australia to inform service
delivery improvement.
- Developing a strategy that includes:
* Dementia awareness and prevention.
* Early identification and timely diagnosis of dementia.
* Quality care and support.
* End of life palliative care, including bereavement care.
* Care for Indigenous populations, people from diverse cultural backgrounds,
younger people with dementia.
* Carers.
(3) No additional funds have been allocated to address planning issues
associated with dementia in 2012-2013. WA Health's Aged and Continuing
Care Directorate has redirected project staff to support the WA Dementia
Working Group. This planning work is ongoing and is supported through the
WA Aged Care Advisory Council.
