(1) The Friend in Need - Emergency (FINE) scheme will deliver care and support to people in need enabling them to remain in their own home, hostel or nursing home, rather than present to an emergency department or be admitted into hospital. The FINE scheme will include a mix of multi?disciplinary services for patents and their carers.
The scheme involves local GPs, and supports GPs in their care of patients in community settings.
The FINE scheme will comprise three interlinked components:

Community based non-inpatient acute and complex care
for patients and carers whose acute and complex care needs can be safely and effectively managed in the community through provision of short term care that is a substitute for in-hospital care.
*
A strengthened network of care coordination (case management) that includes complex care coordination
to help patients and their carers to navigate options of care and support.
*
Community based flexible care packages
, purchased from private providers that are responsive to client and carer needs when mainstream service (such as Australian Government funded Community Aged Care Packages or Extended Aged Care at Home packages) is either delayed or not available.
(2) Care coordination and community-based flexible care packages will commence operation in February 2009.
Community-based non-inpatient acute and complex care has a planned phased-in start at 1 July 2009, and will be fully operational and rolled out to all designated hospital catchments by July 2010.
(3) A provider will be appointed for the community based non-inpatient acute and complex care and community based flexible care packages. Delivery of this metropolitan?wide service will require pre-existing skills and expertise, infrastructure, economy of scale and demonstrated program development experience.
(4) A steering group and working group will provide forums through which emergency departments, GPs and non-government organisations will liaise together on all aspects of the scheme.
