I am advised:
(a) Since 17 March 2017, there were 8 reports received of lost propofol. These 8 reports included a total of 10 ampoules reported as lost.
(b) The facilities named in the reports included, Albany Hospital, Fiona Stanley Hospital, Rockingham General Hospital, Royal Perth Hospital and Sir Charles Gairdner Hospital.
(c) The approximate value of the amount reported as lost is $9.27.
