I refer to the operations known as Kalgoorlie Consolidated Gold Mines Pty Ltd (KCGM), Barrick Raleigh and Barrick Kanowna respectively in the Kalgoorlie-Boulder region, and ask -
(1) Will the Minister state whether or not the above mentioned operations have as a record an accident report book as required by section 77, of the
Mines Safety and Inspection Act 1994
?
(2) If no to (1), why not?
(3) Will the Minister state the specific requirements of section 77, of the
Mines Safety and Inspection Act 1994
in regards to the accident report book?
(4) If the above mentioned operations respectively have an accident report book, does each book comply with the specific requirements of section 77, of the
Mines Safety and Inspection Act 1994
?
(5) Will the Minister state, what is the penalty for non compliance of section 77, of the
Mines Safety and Inspection Act 1994
?
(6) Will the registered manager, or any other person, of the above mentioned operations be prosecuted for failure to comply with section 77, of the

3. The book must be of a type approved by the State mining engineer.
The book must be kept open at all reasonable times to the examination of an inspector, an assistant inspector, a safety and health representative, a representative of a trade union any member of which is employed at the mine and of any other person authorised by the State mining engineer.
4. Yes. Both Kanowna Belle and the Raleigh Gold Mine use a computerised system to record all near misses, incidents and accidents.
A hard copy in the book form is also available.
Permission to use an electronic system was given by the State mining engineer during 1997.
5. For breach of Section 77 relating to Accident Records there is a Level 1 penalty as specified in the
