For each of the Department of Housing Homeswest maintenance
job orders 4631496, 4622621, 4636274, 4696364 and 4624199 —
(1) On what
date was the job order submitted to the Department of Housing for payment and
on what date was the job order paid by the department?
(2) Who informed the department that the job order was
fraudulent or noncompliant and when?
(3) Was the job order payment refunded to the Department of
Housing; and, if yes, on what date?
(4) Was any
contractor or subcontractor associated with the job orders listed, or any of
the other eight job orders identified by the same audit, terminated by the head
contractor; and, if so, which contractor or subcontractor was terminated and on
what date?
