I refer to the Ombudsman’s finding that approximately 24 per cent of police risk assessment responses were recorded as ‘unknown’ or ‘unstated’, and I ask: (a) what proportion of risk assessment fields have been incomplete in each of the past three years; (b) what policies require completion of all risk assessment fields; (c) what quality assurance processes are in place; and (d) has the increase in FDV demand impacted completion rates?

(a) The Western Australia Police Force “police risk assessment responses” are recorded as risk and behaviour questions. Analysis of the WA Police Force records for the past three calendar years provides the following percentage of unstated or unknown risk and behaviour questions:
(b) Policy requires attending officers to complete the risk and behaviour questions unless the person is unwilling or unable to answer them. Any relevant explanation is added in the account field. The relevant policy is contained within the WA Police Force - Police Manual:
(c) The WA Police Force has multiple quality assurance processes for Family Violence Incident Reports (FVIR’s) including:
(d) No
