I refer to the Kanowna Belle gold mine and Raleigh Gold mine, both owned by Barrick Gold located near Kalgoorlie Boulder, and ask -
(1) Has any written directions or communications in correspondence concerning environmental or mines safety matters for the above mines, been issued by the Department of Mines and Petroleum (DMP), for any issues in the years; -
(a) 2008;
(b) 2009; and
(c) 2010?
(2) If yes to (1), what specifically were the written directions and issues raised in correspondence?
(3) In the period for the last 36 months can the Minister state how many prohibition notices have been issued by DMP inspectors for all the above operations?
(4) If yes to (3), can the Minister list all the reasons as to why they were issued?
(5) If no to (3), why not?
(6) In the period for the last 36 months can the Minister state how many prohibition notices have been issued by DMP inspectors and list all the reasons as to why they were issued?
(7) If no to (6,) why not?
(8) In the period for the last 36 months can the Minister state how many improvement and provisional improvement notices have been issued by DMP inspectors for all the above operations and list all the reasons as to why they were issued?
(9) If no to (8), why not?
(10) In the period for the last 36 months can the Minister state how many provisional improvement notices have been issued by DMP inspectors for all the above operations?

Mines Safety and Inspection Act 1994 and Regulations 1995

(17) If yes to (15), how did the DMP factually establish the compliance?
(18) What is the name of the ventilation officer registered with the DMP for the above operations?
(19) Have any concerns been raised in the last 12 months, with the DMP inspectors concerning dust, diesel particulate matter or any other gases along with the primary and secondary ventilation in the Barrick Raleigh mine particularly wet bulb (humidity), dry bulb (dry heat) or ventilation flow (volume)?
(20) If yes to (19), -
(a) what specifically were these concerns;
(b) when on what dates were they raised; and
(c) how were they investigated by the DMP?
