(2) For each of the schools listed at (1):
(a) is the dental clinic housed in a stand-alone building, or is it part of the main school building, and if it is stand-alone, is it a transportable or permanent building;
(b) how many staff work in the clinic and what are their job titles;
(c) by which Government agency is each staff member employed;
(d) if a dentist is not part of the clinic’s staffing complement, under what arrangement is a dentist employed;
(e) what surrounding schools does the clinic service; and
(f) how many students were treated in the clinic during:
(i) 2007;
(ii) 2008;
(iii) 2009;
(iv) 2010; and
(v) 2011 to date?
