representing the Minister for Health:
(1) Will the
Minister for Health table the south west region model of palliative care and
the Perth metropolitan area model of palliative care?
(2) If no to (1), why not?
(3) In the south
west region, is the palliative care telephone advice line number the same as
the general phone number for the local hospital?
(4) How many
nurses working at hospitals in the south west region after hours hold a specialist
certificate in palliative care or a graduate certificate in palliative care?

The metropolitan area model of
palliative care is provided by the Silver Chain Group nursing association.
Silver Chain provides a holistic model of care that includes an in-home,
24-hour, seven‑day-a-week service provided by a multidisciplinary team
that has a core workforce of specialist palliative care nurses supported by
specialist medical staff, general practitioners and a clinical governance
framework.
(3) Across the
south west region, each district has a specialist palliative care nurse who is
supported by the regional palliative team. Each nurse has a dedicated phone
number. Community palliative care patients are advised to call the local
hospital after hours if advice or support is required. Both the local and
palliative care nurses are part of the care team and a clinical handover occurs
for specific patient details, medication schedules and care needs. Terminal
patients have access to in-home palliative care if they wish to be cared for at
home.
(4) General
nurses working in the hospital are not required to hold specialist palliative
qualifications. The hospital teams have access to the specialist palliative
care team during working hours and also have access to an after-hours
palliative care specialist advice line in Perth should they require assistance.
