The majority of assaults have been verbal (related to threats and aggressive behaviour). (2) The investigation into the assault on 25 January 2004 found that the duress alarm failed due to a problem in the installation system as a result of recent upgrades to the paging system. Broome Health Services is currently installing a new improved duress system which will function across all areas of the hospital and enable a number of staff to carry duress pendant alarms. (3) The regular testing of the duress alarm system is the shared responsibility of the hospital management team. The investigation identified several failures in the security management system. The hospital took a no blame approach and has taken immediate action to ensure improved systems are implemented to reduce future risks. (4) Protocols that have been put in place include: - Physical improvements in the areas of lightening, video surveillance and after-hours access. - A new protocol developed and circulated to ensure that nursing staff are not left alone with patients in the Emergency Department. - Staff have been reminded of the existing protocol requiring two people to attend the front door after-hours to admit patients and their rights to refuse service if patients appear to be aggressive or agitated. - Extensive training has been conducted in accordance with the regional Aggression Management Policy which requires staff to ‘not knowingly expose themselves to actual or potential aggression or violence’.
