(2) If the answer to (1) is yes -
(a) on what date was the assessment of the accessibility of each service made;

(c) how was the assessment of the accessibility of each service made?
(3) If the answer to (1) is no -
(a) which services do not have comparable access;
(b) why does the region not have comparable access; and
(c) what steps have been taken to ensure comparable access?
