For those Agencies under the Minister’s control, for the 2008-09 financial years can the Minister advise, -
(1) How many ‘appropriate authority investigations’ into alleged misconduct were conducted by Agencies under your control?
(2) Of the total, how many are complete?
(3) What was the finding from the investigation?
(4) Was a report done on each of those investigations?
(5) If no to (4), why not?
(6) Were these reports made public?
(7) If no to (6), why not?
(8) How many investigations are still ongoing/outstanding?

(1)
The Disability Services Commission conducted 10 investigations into 12 alleged breaches of the WA Code of Ethics and/or DSC's Code of Personal Conduct during the 2008-09 period.
(2)
Nine investigations were completed during 2008-09. The other investigation was completed in July 2009.
(3)The findings of the investigations were as follows -
Four terminations of employment;
Three formal reprimands issued; and
Two findings where no breach was found to have occurred (allegation unsubstantiated).

The reports are made public upon request from the employee under investigation or the WA Police Department. Current practice is that all investigation reports are submitted to the Corruption and Crime Commission without request.

· inappropriate use of Commission IT resources - related to viewing inappropriate material on the internet;
·inappropriate conduct towards colleagues - related to breaches of the Public Sector Code of Ethics;
· mishandling of resident(s) - related to inappropriate incidents involving Commission residents;
· fraud / falsifying documents - related to petty cash declarations and attendance records; and
· fitness for work issues - related to frequent absences from the workplace.
