I refer to deaths of children in the care of the CEO, and I ask: (a) for all children who have died while in the care of the CEO since 1 July 2012, which cases have been reviewed by the Ombudsman of Western Australia; (b) have all cases been subject to a mandated Inquest by the Coroner; (c) if yes to (b), what was the outcome of each Inquest; and (d) if no to (b), when is each Inquest expected to commence?

(a-d) Although all deaths of children in the CEO's care meet criteria for an investigable death by Ombudsman of Western Australia (OWA), OWA does not subject all these cases to a review. It is recommended that OWA be approached directly for this information.
All deaths of children in the CEO's care are subject to a Mandated Inquest by the Coroner. Considerable delays are experienced in Mandated Inquests being held by the Coroner. It is recommended that this information be sought directly from the Coroner's Court of WA.
