Regarding the recently opened Denmark Health Facility, I ask -
(1) What was the total expenditure for the new health facility, including all development, planning and associated costs?
(2) What funds were spent on the following items, -
(a) treatment ponds and drainage;
(b) roads and parking facilities;
(c) bulk earthworks, including fill;
(d) any other site development work;
(e) the buildings including all fixtures and fittings; and
(f) any other costs not included in (a) to (e) above?
(3) What are the names of the contracting companies engaged, directly and indirectly, in carrying out the above works?
(4) Of the contractors identified in (3), -
(a) what individual parts of the construction works has each carried out; and
(b) what funds have been paid or committed to each contractor?
(5) Of the funds identified in (4)(b), which part or parts of the construction works have they been for?
(6) How many cubic metres of fill were delivered to the site?
(7) How many cubic metres of soil were removed from the site?
(8) Where was the soil removed from the site taken?
(9) What funds have been spent or committed in preparing the old Denmark Hospital and Reserve for hand over to the Shire of Denmark?

(2) (a) $732,000;

(c) $2,486,000 including removal of contaminated soils;
(d) $120,000 for electrical mains supply and $87,000 for sewer extension;
(e) $12,894,285; and
(f) nil.
(3) Jaxon Constructions (now trading as Doric Commercial) were directly contracted as head contractor, and also supplied carpentry and general labour. The subcontractors employed by them were:

· Dunsborough Rammed Earth;

(4) (a) Denmark Earthmoving: bulk earthworks, siteworks, building pads, car-parks and roadworks;

Castlehow Electrical: mains electrical supply and building electrics;
Centigrade: mechanical services, medical gases and air-conditioning;
Teksteel: structural steel;
Koster's Steel: metalwork;
Dunsborough Rammed Earth: limestone retaining walls and rammed earth walls;
Nationwide Roofing: roofing and roof plumbing;
Albany Carpet Choice: resilient finishes (carpet and vinyl laying);

(b) The value of work certified to the head contractor to date is $16,924,265. Within this is included amounts for Jaxon Constructions' subcontractors as follows:

Castlehow Electrical: $1,980,000;
Centigrade: $2,235,500;
Teksteel: $850,000;
Koster's Steel: $1,440,000;
Dunsborough Rammed Earth: $370,000;
Nationwide Roofing: $570,000;
Albany Carpet Choice: $405,000;
HK Cabinets: $550,000;
Albany Plasterboard: $600,000;
Caterlink: $260,000; and
Elegant Landscapes: $330,000.

(8) Organochlorine Pesticide Soils have been diluted and taken to the Denmark Shire waste facility.  Acid Sulphate Soils (ASS) have been treated and taken to Denmark Earthmoving's quarry pits. This contractor is required to lodge a
close-out report with the Department of Environment and Conservation verifying their compliance with the ASS Management Plan.
(9) The Department of Health negotiated a land exchange with the Shire of Denmark, with the old hospital site to transfer to the Shire. The old hospital is listed on the Heritage Council's Register of Heritage Places. The Department of Health has advised that $25,000 has been allocated for the preparation of a conservation plan for the old hospital site.
