I refer to Question on Notice 5252 in relation to Cockburn Sound, and ask: (a) can the Minister confirm the total number of unauthorized discharges into Cockburn Sound since 2008 is 11, as provided in the answer to QON 5252; and (b) if yes to (a): (i) why are only eight incidents outlined in part (d) of the answer provided; and (ii) what are the three incidents not outlined in part (d) of the answer provided, including the premise; volume; composition and nature of the discharge, and what action, including prosecution, took place?

(i) The details of three incidents were not provided due to an administrative error.
(ii)
9. (i) 4 February 2014 - CSBP Wharf Kwinana - spillage of an estimated 50kg of fertiliser.
(ii) Investigated - equipment failure. No further action.
10. (i) 20 June 2014 - CSBP Wharf Kwinana – conveyor spillage of 3-5kg of Urea.
(ii) Investigated. Fremantle Port Authority to facilitate procedures to prevent recurrence.
11. (i) 23 November 2015 - Coop Bulk Handling - spillage of an estimated 500kg of Canola grain.
(ii) Investigated - letter of warning issued.
