Answer as at 18 June 2015:
(a)
Yes, it is a permanent position classified as a Level P4 under the
WA Health - Health Services Union (HSU) WA - PACTS Industrial Agreement 2014.
(b) Since 5 December 2011. The acting employee's salary rate is HSU Level P4.
(c) Four.
(d) The panel could not identify a suitable applicant from the recruitment processes held in relation to this position.
