I refer to question on notice No. 1266 regarding a suspected fire on a Fremantle Train, and I ask: (a) has the investigation into this incident been completed; (b) if yes to (a), will the Minister table a copy of the report; (c) if no to (a), why not, and when does the Minister expect this report will be completed; (d) who carried out the investigation; (e) was this incident referred to the Office of Rail Safety; (f) if yes to (e), on what date; (g) if no to (e), why not; and (h) were independent witnesses interviewed and, if yes, how many?

(a) Yes.
(b)-(d) The incident was recorded in the Public Transport Authority's Incident and Fault Reporting System in accordance with the Authority's standard protocols, which then required relevant staff to review the incident to confirm the cause and how the fault would be rectified. Accordingly, railcar maintenance staff investigated the incident and found a wiring fault which was repaired. A separate investigation report was not completed.
