(b) As stated on page 27 of the Annual Report : ‘A form is assigned a status of ‘void’ when a subsequent Consulting Assessment Form is submitted, or a form has been superseded by another valid submission.’

(d) Yes. For a voluntary assisted dying substance to be supplied, a valid form must be submitted at each proceeding stage of the process. Forms which were void on submission were superseded by valid forms and voluntary assisted dying substances were supplied.
(e) As stated on page 27 of the Annual Report : a form is assigned a status of ‘revoked’ when a patient has revoked their administration decision or appointment of a Contact Person.’
