AMBULATORY CARE SERVICES PROGRAM
In question without notice 1094 asked on 21 November 2006, the government referred to rolling out a major program of ambulatory care services. (1) How much funding has been committed to this program in the south west region? (2) When will the government expand the ambulatory care program to each of the following centres of the south west region: Busselton, Margaret River, Manjimup and Bridgetown? (3) When did the government formally take over responsibility for post-acute care in the south west region? (4) How many patients in the south west region have received hospital-in-the-home-care in this state? (5) According to the answer to question without notice 1059 asked on 15 November 2006, funding to the Silver Chain Nursing Association increased in 2006-07 by $3.36 million. In 2006-07, the Ambulatory Care Program committed $125,000 to the Hospital Nurse Discharge Program provided by Bunbury Hospital and $65,000 was allocated to telephone support and coaching. 2. The WA Country Health Service (WACHS) is currently working with the Metropolitan Area Health Services to expand Ambulatory Care Initiatives in 2007/08 across the State, which will include Bunbury in the South West. It has not yet been determined whether any other towns in the South West will be included in the 2007/08 expansion. 3. WACHS - South West has always had responsibility for the post acute care of patients. The Bunbury Hospital Nurse Discharge Program commenced in October 2006. 4. Since the Bunbury Hospital Nurse Discharge Program commenced, there has been 2,496 occasions of service. The current data collection process does not allow the calculation of individual patient numbers only the occasions of service (some individual patients may have several/numerous occasions of service whereas others may only been seen once). 5.
