I refer to the death-in-custody of Aboriginal artist Maureen Mandijarra from Balgo, who passed away while being held in a police cell in Broome in 2012, and I ask: (a) why has the mandatory investigatory report still not been delivered two years after her death; (b) what action will the Minister take to ensure the report is handed down without further delay, and a coronial inquest completed; (c) what has caused, and is still causing, the delay in the completion of the investigatory report; and (d) has the Minister apologised to Ms Mandijarra's family for the delay: (i) if no to (d), why not?

(a)-(d) The completed investigation file was forwarded to the State Coroner on 15 September 2014. The length of time taken for this investigation was affected by the principal investigation being expanded to include the examination and testing of policy with respect to custodial care.
