For each hospital and service provider, as at 16 September 2019, has any medical equipment gone without servicing for more than 6 months after its scheduled/required service date, and if so: (a) What is the medical equipment and where is it located; (b) Is the equipment still operational: (i) If not, why not; (c) What is the anticipated cost of servicing the equipment; (d) Why has it not been serviced; (e) How long has the servicing been overdue; and (f) When is it planned to be serviced?
