I refer to Technical Bulletin 40/1 Discharge Policy in Western Australian Hospitals, and I ask -
For those hospitals that have Mental Health Facilities -
(1) What are the exact accreditation requirements of discharge planning?
(2) Has each hospital been assessed to ensure they meet the accreditation requirements for discharge planning?
(3) If yes to (2), who conducted the assessment?
(4) If no to (2), why not?
(5) If yes to (2), will the Minister table the results of the assessment?
(6) If no to (5), why not?
(7) Which hospitals, if any, do not meet the accreditation requirements?
(8) What action is being taken to bring those hospitals up to the appropriate standard?

All hospitals have been surveyed by the Australian Council on Healthcare Standards (ACHS) using the Evaluation and Quality Improvement Program (EQuIP).The Mental Health discharge planning accreditation requirements are listed under criteria 1.1.5 and 1.1.6.
All hospitals received full accreditation.
