I refer to the Premier's answer to a question of which some notice was given on 23 May 2012 relating to the Premier's attendance at the Leaders' Forum on 14 December 2011, 4 November 2010, 21 May 2010, 26 March 2009 and 28 October 2008, and I ask, for each of those dates, please list the name, role title and salary level of the staff member(s) who attended each event, and the duration of their attendance at each of those meetings?

In some cases, staff who have attended those events listed are no longer employed in the Office of the Premier. As such, it is encouraged that the Member submit a Freedom of Information request to the Office of the Premier so that appropriate considerations can be made with respect to the provision of personal information in those instances.
For the meetings where current staff within the Office of the Premier have attended the information is as follows:
26 March 2009 staff member attending was Mr J Hammond PSM.
21 May 2010 records indicate that no staff member is listed as having attended the meeting.
4 November 2010 staff member attending was Mr B Pontifex.
14 December 2011 staff member attending was Mr J Hammond PSM.
Information regarding staff employment details can be found in the quarterly Ministerial resourcing report, tabled in Parliament.
