(1) An analysis is conducted utilising population data and Home and Community Care (HACC) service provision data to determine gaps in services and to examine trends in HACC target population and HACC client numbers at a regional level.
Consultations are conducted with HACC Program service providers and other relevant stakeholders within each region across Western Australia. The data analysis is used at these forums to inform discussion about unmet need within the program.
Regional priorities are then set utilising the information from the analysis and consultation processes.
(2) The most recent data analysis was conducted in January/February 2013 in preparation for the consultation process.
Consultations to identify need for Home and Community Care services in the four metropolitan HACC regions occurred between 22 February 2013 and 12 April 2013.
