(3) At 8 February 2016, how many non-terminal palliative care patients were receiving support services by the Bunbury Community Palliative Care Service? (4) At 1 September 2016, how many non-terminal palliative care patients were receiving support services by the Bunbury Community Palliative Care Service? (5) Between 22 June and 1 September 2016, how many palliative care patients, or their carers, made telephone contact with the Bunbury Hospital after-hours manager? A specific number will not be provided in order to protect patient confidentiality and decrease the potential for identification of individuals, in accordance with “Guidelines for the Disclosure of Secondary Use of Health Information for Statistical Reporting, Research and Analysis 2015”.
(3) At 8 February 2016, a total of 72 non-terminal patients were receiving support services from the Bunbury Community Palliative Care Team.
(4) At 1 September 2016, a total of 75 non-terminal patients were receiving support services from the Bunbury Community Palliative Care Team.
(5) Between 22 June and 1 September 2016, nil telephone contacts were received by the Bunbury Hospital after hours manager.
