on 4 August 2001 entitled ‘Please Explain Call on Car’ and ask what are the policy guidelines under which Ms Woods was allocated a motor vehicle?
(2) Who is Ms Woods’ Manager / Supervisor and when did he / she become aware of the incident?
(3) What part of the organisational structure does Ms Wood’s belong to and who is the Director /Assistant Director responsible for this part of the structure?
(4) When did the Director/ Assistant Director become aware of the incident?
(5) When was Mr Curry made aware of the incident?
(6) Who made Mr Curry aware of the incident?
(7) What date did Mr Curry write to Ms Woods about the incident pursuant to the Public Sector Management Act 1994?
(8) Was Mr Davies, Chairperson of ATSIC’s Regional Council Geraldton, authorised to drive the vehicle?
(9) Was it lawful for Mr Davies to be driving the vehicle of the Indigenous Affairs Department (IAD)?
(10) If so- under what provisions of the department’s motor vehicle policy?
(11) Was it unlawful for Mr Davies to be driving the motor vehicle of the ( IAD)
(12) If so has there been a breach of any State Legislation?
(13) Has the Anti Corruption Commission been advised of the incident?
(14) If not, why not?
(15) On what date did Ms Woods make payment to the service station?
