I refer to the Ombudsman’s findings that systemic issues in the family and domestic violence response system have persisted despite ongoing reviews since 2012, and I ask: (a) what governance and oversight structures are currently in place to monitor the performance of the family and domestic violence system; (b) how accountability is shared and coordinated between the Department of Communities, WA Police and other relevant agencies; (c) what actions have been taken to address recurring issues identified in Ombudsman reviews since 2012; and (d) what explanation has been provided for the persistence of similar systemic failures across successive reviews?

(a – d) Oversight of the Family and Domestic Violence Response Teams (FDVRT) is provided by an Executive Steering Committee, supported by an Operational Working Group. Both groups comprise representatives from partner agencies.
Issues identified by the Ombudsman are actively addressed and monitored through existing governance arrangements. Reforms initiated and oversighted through these arrangements include:
The Ombudsman’s Own Motion Investigation examined data from a time period prior to substantive implementation of these reform improvements. The FDVRT model continues to evolve as improvement opportunities are identified.
