(2) Each organisation will receive funding attached to the particular individuals they are matched with. Funding allocated to each individual varies in accordance with the bandwidth of support identified as appropriate by clinical services. The bandwidths are Low ($0-$30K), Medium ($30K-$60K), High ($60K-$90K) and Very High ($90K to $120K).

(5) Supports and services will be identified through a holistic planning process. Each individual's personalised plan will identify personal goals and outcomes focused on personal recovery, as well as clinical support needs. The funding will be utilised to provide direct support through the identified community support organisation. The funding can also be used to broker specific support required by an individual where the support organisation does not have expertise in that area (eg drug and alcohol counselling) or to access community activities where these are in accordance with the person's individualised plan. See (14) below for details of clinical support.
(6) Individuals are being introduced to the key staff of the community support organisation to initiate the holistic planning process with input from the person, family and other key supporters as well as the current clinical team.
Formal support will commence on following the endorsement of the individual's plan by the independent panel. The first panel sitting is anticipated in May 2012.
(7) Each person will be integral to the developing of their personal support plan. This will include what supports they identify as important, how these are provided and by whom.
(8) Each person has been involved in identifying the community organisation to coordinate and provide their support. Some individuals specifically identified organisations, some identified organisations they did not want to provide support and some indicated no preference. Where no preference was indicated, locations of organisations and style of support were taken into consideration as part of a matching process.
Funding is portable and if required the support funding is able to be transferred to an alternative organisation (as identified through the tender process).
(9) 98 individuals are currently identified.
In addition (taking the total to 100);
· One individual previously identified has recently withdrawn from the process and is being replaced.
· The process for one individual identified is on hold. The individual is currently in custody with trial date in April pending. The outcome of the trial will determine whether the place on the program is held (custodial - dependent on length / non-custodial sentence).

(11) People will not be funded for particular hours of support. Each person will have a plan developed around their particular needs and supports will be identified accordingly. Funding will be allocated within the identified bandwidth of support in accordance with their individualised plan.
(12) Approximately half of the 100 people have identified that they currently and will continue to receive supports through their family and/or friends.
(13) Each individual has identified their preferred location to live. Proximity to family, friends, preferred community activities and currently utilised services such as clinical supports has been a major consideration in discussions prior to people identifying their chosen locations.
(14) Specialist clinical support will be an ongoing commitment. Current inpatient settings will provide a thorough handover to community clinical teams and a detailed transition plan will be developed that is inclusive of the supports to be provided from the community support organisations. Transition for some individuals will be lengthy and will start some time before they are ready to move permanently into their new home.
