The Driver's certificate was issued prior to the implementation of the National Certification Standard which ocurred in Western Australia on 1 July 1994. To attain the Driver's Certificate of Competency, the driver would have completed the relevant training at his place of employment and then been examined by an Inspector employed by the then Department of Occupational Health Safety and Welfare (DOHSWA). (ii) Date of Incident - 14 May 2002. Certificate Issued - 30 August 1985. The Driver's Certificate of Competency was issued prior to the implementation of the National Certification Standard in Queensland which occurred in the mid 1990's. (iii) Date of Incident - 17 May 2002. Certificate Issued - 20 June 2000. Registered Training Organisation Name - Construction Skills Training Centre. Assessor - Ken Sully. (iv) Date of Incident - 30 May 2002. Certificate Issued - 4 July 1979. The Driver's certificate was issued prior to the implementation of the National Certification Standard which occurred in Western Australia on 1 July 1994. To attain the Driver's Certificate of Competency, the driver would have completed the relevant training at his place of employment and then been examined by an Inspector employed by the then Department of Occupational Health Safety and Welfare (DOHSWA).

(a) & (b) Training for crane operators is undertaken by Registered Training Organisations who are accredited through the vocational and education training sector. Such a list is not maintained by the Department of Consumer and Employment Protection.
