For the period 23 September 2008 to 31 July 2009, could the Minister please advise for each agency within his portfolio:
(a) has any officer above Level 9.1 within the agency accepted any free or discounted service from a private company or individual;
(b) if yes to (a), how many officers above Level 9.1 have accepted such free or discounted service from a private company or individual;
(c) if yes to (a), what was the nature of the free or discounted service and what is the name of the individual or private company that offered the free or discounted service;
(d) if yes to (a), what is the estimated value of the service provided; and
(e) if yes to (a), does the agency have any commercial or financial relationship to the private company or individual?
