I refer to the alleged sexual
assault of a 13-year-old in ward 5A at Perth Children's Hospital in
January 2022 and the 11 recommendations contained in a report prepared by the
Office of the Chief Psychiatrist following the horrific incident.
(1) How many recommendations have
been fully implemented?
(2) How many recommendations have
been partially implemented?
(3) How many recommendations have
not commenced?
(4) Have any changes to the ward
environment been completed; and, if yes, what changes?
(5) How many
patients are currently in ward 5A by sex and what are the new staffing levels
of the ward?

some notice of the question. The following has been provided by the Minister
for Health.
(1)–(4) Of the recommendations, 10 have been fully
implemented and one recommendation remains in progress. Since 2022,
various safety and security changes to ward 5A have been completed including
additional CCTV cameras, upgraded door hardware, glass panel replacement,
additional viewing panels for the seclusion room, the removal of corridor wall
bumpers, a trial of a new ensuite door in one ensuite, reinforcement of staff
station joinery panels, locks on all fire hydrant cupboards and the replacement
of damaged furniture.
Following extensive consultation
with stakeholders and consumers with lived experience, a broader program of works is currently underway for ward 5A,
with forward works construction beginning in May 2024. To ensure ward 5A
continues to meet the contemporary needs of its patients, 12 of the ward's 20 mental health beds will be moved to a refurbished
ward leased onsite at Hollywood Private Hospital, to allow for infrastructure
upgrades to take place sooner. Remaining beds will be available for children up
to 13 years old. The 12 beds at Hollywood Private Hospital will continue to be
staffed by existing experienced child and adolescent mental health
clinicians employed by the Child and Adolescent Health Service.
(5) Patient
information will not be provided. The ward currently has 17 patients. The ward
5A staffing profile is a large
multidisciplinary team. The current nursing staff levels are above nursing
hours per day allocation , and additional staff are included according to
acuity and risk.
