(1) When and how many safety-related inspections or visits have been carried out by the Department of Mines and Petroleum’s resource safety officers at this operation in the past five years? (1) Sixteen inspections were carried out between 26 September 2006 and 28 July 2011. These occurred on: 26 September 2006; 2 October 2006; 20 November 2006; 16 May 2007; 19 June 2007; 16 October 2007; 29 November 2007; 27 May 2008; 22 June 2008; 10 November 2009; 8 March 2010; 22 April 2010; 21 June 2010; 14 September 2010; 27 July 2011; and 28 July 2011. (2) As a result of these inspections, 12 improvement notices were issued. (3) These were issued as follows: on 2 October 2006 four notices were issued. The information on these has been archived, and details will be forwarded once sourced. On 16 April 2007 one notice was issued in relation to dust control. On 19 June 2007 two notices were issued in relation to guarding on a grinder, and provision of a charging station for a forklift. On 16 October 2007 one notice was issued in relation to an eyewash station. On 29 November 2007 one notice was issued in relation to emergency lights and signage. On 22 June 2008 one notice was issued in relation to dust control. On 22 June 2008 one notice was issued in relation to noise. On 22 April 2010 one notice was issued in relation to roadside reflectors.
