(2) How many of the individuals identified in (1), requested an early review of their plan? 1. Plans are reviewed every 12 months, or more frequently if required, updating the goals and strategies in response to any changes in an individual's circumstances.

3. Self-management of an individuals' plans carries considerable responsibility and sometimes individuals and families find the process too time consuming or complex after an initial trial period. Individuals are free to change their plan-management option to best suit their needs, capabilities and current circumstances.
