RESPECTING PATIENT CHOICES - ADVANCE CARE PLANNING PROGRAM
(1) What level of state government funding was provided to Fremantle Hospital to pilot the Respecting Patient Choices advance care planning program in 2005-06, 2006-07 and 2007-08? (2) What level of federal government funding was provided to Fremantle Hospital to pilot the Respecting Patient Choices advance care planning program in 2005-06, 2006-07 and 2007-08? (3) Has a business case been presented to the Department of Health and/or the minister for continued funding of this program for 2007-08? The total amount was approximately $56 000 plus on-costs. (2) From October 2005 to October 2006, the commonwealth Department of Health and Ageing funded the project - namely, $165 000, including GST.
