The Department of Communities (Communities) advises:
(a - b)
Communities takes all incidents of self-harm seriously. Local and individualised responses are actioned as a matter of priority; these may include safety planning, district psychologist consultations, and appropriate referrals or admissions for specialised intervention and/or support.
(c)
The Coroners Court of Western Australia is responsible for determining the cause of death of any child or young person in care. In the last five financial years, the Coroner has reported zero suicides relating to children or young people in the care of the CEO.
