For each agency and department under the Minister’s authority, have there been any privacy breaches identified in which the department or agency disclosed personal information without the consent of the individuals whose information was disclosed: (a) if so, what was the nature of the breach and what action was taken subsequent to the breach being identified; (b) was the breach disclosed publicly, and if so, how and if not, why; and (c) were all individuals whose information was disclosed advised of the breach, and if so, how and if not, why?

It is not considered a reasonable or appropriate use of Government resources to compile a breakdown across the entire history of each agency.
