(1) How many Critical Incident Notifications have been issued since the beginning of the 2025–26 financial year? (2) For each notification identified in (1), please provide a breakdown by: (a) the custodial facility where the incident occurred; (b) the type of incident (e.g. riot or disturbance, escape or attempted escape, death in custody, serious self-harm, assault, infrastructure failure, etc.); and (c) the date on which the notification was issued? (3) For each of the financial years 2021–22, 2022–23, and 2023–24, how many Critical Incident Notifications were issued, broken down by: (a) the custodial facility where the incident occurred; (b) the type of incident (e.g. riot or disturbance, escape or attempted escape, death in custody, serious self-harm, assault, infrastructure failure, etc.); and (c) the month and year in which the notification was issued? (4) Of the notifications recorded since 2021–22, how many related to incidents involving serious injury to staff, and how many related to incidents involving serious injury or death of prisoners? (5) For each of those incidents, were referrals made to WorkSafe or the Coroner?

Compiling this information would demand diverting staff away from their operational duties and as a consequence, it is not considered to be a reasonable or appropriate use of resources. I note the Member asked 17 different questions of similar nature in length concurrently.
