(a) No operator’s manual for a forklift;
(b) Bench grinder with no guard;
(c) Unsecured compressed gas bottle;
(d) No lock-out/tag-out system in use;
(e) An incomplete register of hazardous substances;
(f) No written reports of risk assessments for the use of hazardous substances;
(g) Training provided to employees on the use of hazardous substances was incomplete; and
(h) No written records of training provided on the use of hazardous substances.
