The Fire and Emergency Services Authority of Western Australia (FESA) advises:
1. The reporting person completes an occupational safety and health (OSH) Hazard/Accident Report form and forwards it to their supervisor for action. The supervisor discusses the report with the reporting person and the OSH Representative and implements controls where possible. If the controls are not within the supervisor's area of responsibility, the report is forwarded up the chain of command. If the issue cannot be resolved in a timely manner the issue can be referred to the Fire and Rescue Service OSH Committee for resolution. The elected safety and health representative also has the option of issuing a provisional improvement notice. Feedback is given to the reporting person via the manager.
