I refer to an email dated June 24 2003 with further video evidence and three attachments sent to the Minister, copied to the Premier of WA concerning dangerous and unsafe work practices being carried out on June 20 2003 concerning Kalgoorlie Cement Works operated by Mr Mark Posa -
(1) Has the Minister received a copy of this email?
(2) If yes to (1), on what date was it received?
(3) Can the Minister explain the reasons why the ‘Significant Incident Summary - Use of a Forklift with an attachment-25/1996’ was produced on the WA Governments safetyline website?
(4) If no to (3), why not?
(5) Will the Minister ask Worksafe to request a complete copy of the video footage depicted on June 20 2003 of dangerous and unsafe work practices when carrying out an investigation in Kalgoorlie on June 26 2003?
(6) If no to (5), why not?
(7) Will the Minister or Worksafe prosecute the owner operator for dangerous and unsafe work practices as depicted in the video footage dated June 20 2003 to send a clear message that the Government is seriously committed to cracking down on dangerous worksites and dodgy bosses given that the owner has been previously issued with many improvement notices and a number of prohibition notices?

WorkSafe publishes the summaries on its SafetyLine website as well as distributing printed copies to relevant industry bodies, unions, training providers and interested individuals. SIS 25/1996 was produced in relation to an incident involving a forklift with a jib attachment.
