How many critical incidents have
been notified at each of the following hospitals in the months January 2015 to
April 2015 inclusive —
(a) Royal Perth Hospital;
(b) Sir Charles Gairdner Hospital;
(c) Fiona Stanley Hospital;
(d) Rockingham General Hospital;
(e) Armadale–Kelmscott Memorial Hospital; and
(f) Swan District Hospital?

I thank the honourable member for some notice of this
question. I have a bit of detail here so that the member can understand the
answers given for each hospital. ''Critical incidents'' have been
interpreted as those clinical incidents that have been reported to the online
clinical incident management system as a severity assessment code 1 incident,
SAC 1—that is, an incident resulting in, or with the potential to
result in, serious harm or death of a patient. Following reporting of SAC 1 clinical
incidents into the online clinical incident management system, senior staff
perform a review of the incident and confirm whether or not it meets the
criteria for a SAC 1 clinical incident. If the incident is confirmed by senior
staff as a SAC 1 clinical incident, it is notified to the patient safety
surveillance unit of the Department of Health. The number of reported SAC 1
incidents for January 2015 to April 2015 inclusive are —
(a) 33 incidents notified at Royal Perth Hospital—nine
confirmed to date;
(b) 10 incidents notified at Sir Charles Gairdner Hospital—six
confirmed to date;
(c) 24 incidents notified at Fiona Stanley Hospital—nine
confirmed to date;
(d) 23 incidents notified at Rockingham General Hospital—18
confirmed to date;
(e) 20 incidents notified at Armadale–Kelmscott
Memorial Hospital—14 confirmed to date; and
(f) two incidents notified at Swan District Hospital—nil
confirmed to date.
This data includes incidents notified by hospitals that
relate to community health services that fall under the jurisdiction of that
hospital.
