I refer to question on notice No. 198 asked on 6 August 2013 by Hon Robin Chapple regarding the Harlequin mine and the new Sir Major decline, and ask: (a) was the deceased body removed from the mine
prior to the attendance of Department of Mines and Petroleum (DMP) officers; (b) if yes to (a), is this normal practice; (c) at what time is it estimated that the deceased
suffered the fatality on 15 February 2014; (d) at what time and date did DMP officers arrive at
the Harlequin mine site; and (e) at what time and date did DMP officers enter the
Sir Major decline?
