(a) North Metropolitan Area Health Service;
(b) South Metropolitan Area Health Service;
(c) Child and Adolescent Service;
(d) Pathwest Laboratory Medicine Western Australian; and
(e) Dental Health Services,
as at each of the following dates, -
(i) 30 June 2007;
(ii) 30 June 2008;
(iii) 30 June 2009; and
(iv) 24 September 2009?
