This information is not recorded centrally in the Department's client information system. Decisions regarding formal assessments are recorded in case files.
The Department's policy states that where a family presents on two or more occasions (including for reasons of financial assistance) and chronicity becomes a factor, a safety and wellbeing assessment of the family must be undertaken.  This will involve accessing previous intake and contact information.  Where an assessment is not undertaken, the rationale for the decision must be recorded and approved by the designated senior officer.
