STANLEY HOSPITAL — HOT WATER JUNCTIONS
1008. Hon SUE ELLERY to the
parliamentary secretary representing the Minister for Health:
(1) Has a report
been undertaken regarding a review of the incidents that led to the burst water
pipes at Fiona Stanley Hospital?
(2) If yes to (1), will the Minister
for Health table a copy of that report?
(3) Were the hot
water junctions—also known as bellows—that were installed
consistent with high-density polyethylene standards?
(4) Were the hot
water junctions—bellows—installed by staff qualified or
accredited to do so consistent with high-density polyethylene standards?
(5) What has been
the outcome of those inquiries?

I thank the honourable member for some notice of the
question.
(1) An
independent review is currently being undertaken. A report will be produced
following the review.
(2) The report has not yet been finalised.
(3) The
managing contractor was responsible for the design, installation and
commissioning of the hospital. The contract required the managing contractor to
undertake work in accordance with all relevant standards.
(4) All
subcontractors were employed by the managing contractor who was responsible for
ensuring they were appropriately qualified.
(5) The
independent review is not yet complete. A full and detailed bellows replacement
program and risk mitigation strategy has been developed and includes plans to
keep the hospital fully operational during the replacement period.
The bellows replacement will occur
over a number of days. The facilities manager will replace the bellows. All
works will be certified by Brookfield and Eagle Bergman, the supplier of the
replacement bellows.
